Student Request Forms

The Staff at Cleary Lake Veterinary Hospital believes in education. We are not only educated ourselves but also take great pleasure in helping educate others interested in the area of veterinary medicine. With the high demand of students inquiring about veterinary medicine we now ask that each student fill out the forms provided and return it 60 days prior to his or her visit. The information that you provide can help us decide if Cleary Lake Veterinary Hospital is the right fit for you. Call backs are not made until all completed forms have been received. Failure to complete forms is regarded as failure to follow directions. This is taken into account for processing.
